We are now seeing patients by appointment only due to Covid 19. In order to keep our costs at a minimum and our prices low, we need to fill out your paperwork prior to your appointment.Please click the accept terms link below to provide the information we need to fill out your paperwork in advance and schedule a time to see your pet.  

     Appointment scheduling terms:

     1. If you have multiple pets please fill out the questionnaire for each pet.  This information goes directly on your paperwork - vaccine certificate/receipt.

     2. If you are filling out the paperwork for someone else, please make sure to place their address and phone number in the information blocks.

     3. If you would like to purchase heartworm preventative, you must have a current heartworm test with Animal House.  No exceptions!
